Participation acknowledgment

Before submitting registration

The registering adult confirms that the athlete has permission to participate and that the contact, emergency, grade, and health information submitted is accurate to the best of their knowledge.

Volleyball and athletic training involve physical activity and risk of injury. The registering adult agrees to tell Club Xtreme about relevant medical conditions, allergies, restrictions, or emergency instructions and to update that information when it changes.

This online acknowledgment records registration intent. It is not the club’s complete liability waiver. A parent or legal guardian must review and sign the full waiver before the athlete participates.

Registration and payment

A submitted registration does not guarantee placement on a particular team. Club Xtreme staff confirms eligibility, capacity, team assignment, payment status, and participation approval.

If a session reaches capacity, staff may offer another session or a waitlist. Payment is confirmed only after the payment provider or authorized Club Xtreme staff verifies receipt.
